General Comments
Due to state of kitchen and failing score, owner has agreed to voluntarily shut down until corrections are made.

When all corrections have been made, an inspection must be conducted before approval to reopen can be granted. Facility must score a 95 or better or facility will not be allowed to operate.

This facility is not allowed to operate until the above criteria is met.
